درمان زخمهای دهانه رحم  

در ادامه مطلب قبلی با توجه نگرانی های زیاد بیماران  در مورد وجود و نحوه درمان زخمهای دهانه رحم بطور کلی خاطر نشان میشود بیشتر ضایعات موجود معمولا نیاز به اقدام درمانی ندارد و فقط انجام پاپ اسمیر و یا کولپوسکوپی برای رد وجود هر گونه تغییرات بدخیمی یا پیش بدخیمی ضروری است   

در صورت نیاز به درمان نیاز از روشهای کوتر ( حرارتی ) یا اصطلاحا سوزندان دهانه رحم - فریز یا اصطلاحا منجمد کردن دهانه رحم  استفاده میشود  

جدیدا دیگر این روشها به ندرت بکار میروند و معمولا از روش کوتر شیمیایی و با استفاده از اسید موضعی میتوان سلولهای اسیب دیده را سوزاند تا بحای انها سلولهای سالم طبیعی تولید گردد. 

برای اینکار از ترکیبی از اسیدهای خاص استفاده میشود که  با نام تجارتی محلول سولکوجین  در دسترس است.

کولپوسکوپی


تعریف کولپوسکوپی: Colposcopy

 

 " زخم دهانه رحم " اصطلاحی عامیانه است که برای بیان تغییرات ظاهری در دهانه رحم بکار برده میشود . این تغییرات بیشتر اوقات در نتیجه عوامل خوش خیم مانند التهاب و عفونت ایجاد میشود ولی گاهی میتواند پیش در امد بروز سرطان دهانه رحم گردد

کولپوسکوپی یک معاینه بینایی اختصاصی واژن ، سرویکس و گاهی اوقات لب های خارجی و نواحی ولو می باشد. این تست وقتی انجام میشود که پاپ اسمیر تغییرات سلولی غیر طبیعی را نشان دهد واین کار به پزشک نیاز دارد تا از داخل کولپوسکوپ که روی یک میله قرار گرفته است ، ببیند


کولپوسکوپ به پزشک کمک می کند تا با آن مشکلاتی را که اغلب خیلی کوچک و ریزند که روی سرویکس و واژن هستند را چک کند که ممکن است با معاینه منظم و مرتب Missed شوند

اگر چیز غیر طبیعی (آتومالی) دیده شود نمونه ای کوچک بافتی که بیوپسی نامیده میشود معمولا انجام می گیرد . بیوپسی اطلاعات مهمی به پزشک می دهد تا اگر درمانی لازم هست ، تصمیم گیری کند. بیوپسی ها ممکن است باعث کرامپ خفیف شوند

آزمایش پاپ اسمیر را فراموش نکنید

یکی از ترسناک ترین زمانها در زندگی هر خانم . زمانیست که پزشک او ابراز می نماید که جواب پاپ اسمیر او غیر طبیعی می باشد. 

اگر چه بیشتر بیماران فکر می کنند پاپ اسمیر غیر طبیعی مساوی با داشتن سرطان دهانه رحم می باشد ولی در حقیقت اینطور نیست و در اکثر اوقات این حالت در اثر طرایط خوش خیم دیگر رخ میدهد و شایعترین علت آن معمولا در اثز التهاب و عفونت می باشد. 

در واقع پاپ اسمیر یک تست بیماریابی (اسکرینینگ) می باشد و تست تشخیصی( دیاگنوستیک) نیست و در صورت مثبت بودن جواب آن باید با آزمایشات دیگر این پاسخ تائید گردد. 

جهت اینکار  از وسیله ای بنام کلپوسکوپ استفاده میشود که متخصص زنان با این وسیله نگاه دقیق تری به دهانه رحم می نماید 

کلپوسکوپی در اصل مشاهده سرویکس( دهانه رحم با لنز و بزرگنمایی 40 تا 100 برابر می باشد و لذا عملی بیخطر و بدون درد می باشد و بدین ترتیب از نقاط مشکوک نمونه بیوپسی گرفته میشود تا بار دیگر این نمونه در زیر میکروسکوپ مشاهده و پاسخ بطور قطعی تعیین گردد.

Cervical cryotherapy (also called "cryosurgery" or "cryoablation") is a procedure that removes abnormal cells from the cervix. It’s most often done in the doctor’s office, and only takes a few minutes. During cryotherapy, tissue that includes the abnormal cells is frozen. This tissue is destroyed. The tissue that grows back is usually normal. In most cases, all abnormal cells are removed with only one treatment.

Reasons for the Procedure

Cervical cryotherapy may be done if a Pap test and colposcopy show abnormal cells on the cervix. Here are some of the problems cervical cryotherapy treats:

  • Dysplasia occurs when cells in the cervix change in ways that aren’t normal. This is often caused by a sexually transmitted virus called HPV (human papillomavirus).

  • Warts (condyloma) can grow on the cervix. These are also caused by HPV.

  • Chronic cervicitis is a recurring inflammation of the cervix.

 

What Are the Risks?

Risks of cervical cryotherapy are fairly low. They may include:

  • Bleeding

  • Infection

  • Scarring of the cervix

  • The need for further treatment

Getting Ready for Cervical Cryotherapy

Cervical cryotherapy is most often an office procedure. It will be scheduled for a day when you’re not having your menstrual period. You may be asked to sign a consent form before the procedure. You may also be given special instructions, including the following:

  • Avoid sexual intercourse, tampons, douching, and creams or other vaginal medications for a few days before your procedure.

  • Take an over-the-counter pain reliever an hour before your appointment.

  • Urinate before the procedure.

During Cryotherapy

  • You will be asked to lie on an exam table with your knees bent, just as you do for a Pap test.

  • An instrument called a speculum is inserted into the vagina to hold it open.

  • A local anesthetic may be used to numb your cervix.

  • A thin probe is held on the cervix. This creates a ball of ice that freezes and kills the tissue. This step may be done a few times.

  • You will likely have cramps while the ice is touching your cervix. Tell your healthcare provider if the cramps are too uncomfortable.

  • You may feel lightheaded. This will go away when the procedure is over. You can rest on the exam table until you feel better.

After the Procedure

  • You will have a thin, watery discharge after treatment. You may also have some light bleeding.

  • Next, you will have a thicker, yellowish discharge as your body sheds the dead tissue. This may last up to 6 weeks.

  • Use pads, not tampons, immediately after the procedure. Ask your healthcare provider when it’s okay to use tampons again.

  • The cervical tissue can bleed easily as it heals. To protect it, you may be told not to have sex, douche, or exercise for a certain amount of time.

Call Your Healthcare Provider If You Have:

  • A fever over 101°F

  • Abdominal cramps or pelvic pain for more than 24 hours

  • Foul-smelling discharge

  • Heavy vaginal bleeding


Follow-Up

You can probably return to your normal routine right after cervical cryotherapy. Follow your healthcare provider’s instructions while your cervix is healing. You’ll likely have a follow-up appointment 4 to 6 weeks after treatment to make sure you’re healing well. At a later visit, another Pap test or colposcopy may be done to ensure that treatment was successful.
